It was either made by Celebrimbor (Grandson of Feanor and son of Curufin) for Galadriel because he loved her... Or it was made by Enerdhil a smith from Gondolin who gave it to Idril, who in turn gave it to Earendil who brought it with him to Valinor.... And when Olorin (Gandalf) came to Middle Earth he brought it with him....

Out of the original seven: 4 being in gondor, 3 in arnor
At the time of lotr,
-One was possessed by Saruman
-One was possessed by Denethor
-One was possessed by Sauron
-The osgiliath stone was lost into the anduin before the war of the ring
-the Amun Sul stone and the Annuminas stone were drowned with King Arvedui after the fall of arnor
-the last stone was kept by cirdan and the lindon elves until taken back to Valinor

pretty sure that's it, correct me if i'm wrong
